Type II achalasia can be managed by dilatation, Heller myotomy and POEM. Pneumatic Dilatation is perfect for type II achalasia and young age with success nearly 90%. Still main problem is the high possibility of recurrence expectedly within 5 years. Heller myotomy is another good option if done with well experienced hands as overcorrection can cause dysphagia. Finally, POEM is the best for total resolution of the achalasia. Choice of management is partly dependent on you, according to what i already mentioned. Easy and rapid solution is dilatation (drawback: recurrence). Longer efficacy: POEM. Well experienced staff: Heller myotomy.
